I received an invoice for a small quantity generator monitoring fee but I no longer generate hazardous waste. What do I do? |
Write CANCEL on the invoice and mail it to DEQ. Invoices cannot be canceled based on a phone call. You will also need to change your notification of regulated waste activity form or RCRA Subtitle C Notification Form (EPA Form 8700-12) to reflect your current generator status. I received an invoice for a small quantity generator monitoring fee but I have idea what is about. |
In order to ship hazardous waste including spent solvents being recycled, you are required to have an EPA ID No. Your disposal site may have completed the form for you, but you are responsible for paying the annual monitoring fee. The monitoring fee covers the year in which the invoice is issued. For example, the 2009 invoices were mailed in late December, 2008 with their delivery anticipated in early 2009. |

To change information you will need to change your notification of regulated waste activity form or RCRA Subtitle C Notification Form (EPA Form 8700-12) to reflect your current information. The form may be downloaded from DEQ's Web Page at http://www.deq.state.ok.us/lpdnew/forms/indexhazforms.html . EPA will not allow this information to be changed based on a phone call. |

| 4. | My company is no longer at the address listed on the form. How can I change this? |
If your company has moved, you will need to get a new EPA Id No. for the new location. To do this you need to get the notification of regulated waste activity form or RCRA Subtitle C Notification Form (EPA Form 8700-12) from DEQ's Web Page, download and complete it. The form may be downloaded from DEQ's Web Page at http://www.deq.state.ok.us/lpdnew/forms/indexhazforms.html . EPA will not allow this information to be changed based on a phone call. You should also complete a form for the old location indicating you have moved. |

What are the reporting requirements for small quantity generators? |
Oklahoma has no reporting requirements for small quantity generators nor conditionally exempt small quantity generators unless they become large quantity generators. |
