Do
you have an environmental complaint?
A complaint is defined as information provided to
DEQ alleging site-specific environmental pollution.
There are several ways that you can register a complaint.

You may register a complaint by contacting our statewide Environmental Complaints Hotline at 1-800-522-0206. This number is toll-free and is answered 24 hours per day, 7 days per week.

You may also register a complaint by filling out the on-line complaint form and submitting it electronically to DEQ.

For DEQ to adequately address your complaint, we need for
you to provide certain information. Please describe your complaint in as much
detail as possible. Include information about when you first noticed the problem, how often
it occurs, and specifically where it is from, if known. If you do not have an address
for the complaint site, finding directions beginning at a major intersection will
be fine. Be sure the finding directions are complete and, when possible, please use compass directions (north, south, east, west).
Example: from the intersection of Highway X and Z St. go east 2 miles to the stop sign, turn north and property is the 2nd house on the west side of the road.
There are three ways that complaints are received by DEQ and you are
not required to leave your name, address, and phone number if you would rather not. If you
prefer, you can leave your name, address, and phone number. However, complaints on public water and sewer systems cannot be made anonymously or confidentially. To ensure the complaint is addressed properly, it is necessary that the water or sewer system know the location of the problem so that it can be corrected.

A complaint can be received as an anonymous complaint. Your contact information (name, address, and phone number) is not known to DEQ. However, this also means that you will not received updates on the status of the complaint investigation and no information can be given out over the phone.

A complaint can be received confidentially. This means that you have provided your contact information to DEQ, allowing our investigators the ability to contact you for more information if necessary and to keep you updated on the status of the complaint. Your contact information will NOT be released to anyone, except our investigators, unless ordered to do so by District Court. If you would like to submit your complaint confidentially, please fill out your contact information and then check the box to the left of "Confidential" on the form.

If you do not wish to be anonymous and do not care if your information is kept confidential, then you may submit your complaint as unrestricted. Simply complete the form and submit it to DEQ.
